Consent Form must be filled out fully prior to any tattoo or piercing procedure. Your artist will review before your appointment and keep on file. A new consent is needed for each tattoo or piercing.  

1) Appointments are preferred, but walk ins are welcome!
2) A consent form is need for each visit- no exceptions. 

3) DO NOT ENTER IF YOU HAVE BEEN  SICK OR AROUND SICK PEOPLE
4) Only the person getting worked on will be allowed in the station/building. 
5) Please wear a mask if you are able.
